SOCIAL DETERMINANTS PROGRAMS

  • Weight Loss - Dr. McNeill prescribes a structured lifestyle plan that focuses on dietary modifications with the goal of losing weight. This program includes bimonthly weigh-ins with personalized coaching provided by Dr. McNeill and his Certified Medical Assistant.   • Type II Diabetes Lifestyle Management program - Dr. McNeill prescribes a structured lifestyle plan that focuses on dietary modifications and exercise with the goal of improving hemoglobin a1c control and reducing need for medication. This program includes bimonthly weigh-ins, with personalized coaching provided by Dr. McNeill and his Certified Medical Assistant.   • Food Access - Many adults struggle to afford the food that they need. We partner with the YMCA of Asheville through their Nutrition Outreach program to help our patients get access to healthy food. BEHAVIORAL HEALTH ADVANCED CARE TEAM / CO-MANAGEMENT AGREEMENT

Dr. McNeill has a co-management agreement with Linda Newman, LCSW. This means that Ms. Newman and Dr. McNeill agree to collaborate on shared patients regarding the management of their mental health and reduce fragmentation of care.
